Update details: Content of this file was updated to improve the functionality of the form by making further use of select boxes which have been pre-populated with sample content.
The top section of the form allows details of other relevant documents to be entered and select box now in place for 'Type'. Risk rating select box also added to make it easier to assign a risk rating.
Version 2 of this form, which includes a 2nd page with site sign-in sheet, also updated.
