HAVS Forms

Update Details: A number of new forms for assessing, planning and monitoring the use of vibrating equipment where there is a risk of harm from Hand-Arm Vibration (HAVS).

This update includes 8 new forms in total although one form is just 2 of the forms merged into a single form to assist with having all the relevant information on a single document.

Overview of the new forms;

  • F-HS45A HAVS Assessment - form for pre-job planning and detailing vibrating equipment with risk rating and safe operating time
  • F-HS45B Hand-Arm Vibration Use Record - this form can be used by a single worker to log their use of vibrating equipment over a period of time
  • F-HS45C Hand-Arm Vibration Use Record - this form as as per F-HS45B but does not have a name so can be used to log data of multiple workers.
  • F-HS45D Vibrating Equipment Register - register detailing all vibrating equipment
  • F-HS45E HAVS Medical Screening Form - this form can be used to complete a medical screening with a worker prior to them using vibrating equipment
  • F-HS45F HAVS Register - this form is a combination of the F-HS45C and F-HS45D 
  • F-HS45G Hand-Arm Vibration Job Record Form - form for a single job detailing all vibrating equipment on site for job rotation and monitoring
  • F-HS45H HAVS Medical Monitoring Form - this form is similar to F-HS45E but is for use with own workers who are already using vibrating equipment
