HR Forms

Update Details: A number of new HR-related forms have been prepared and added.

The following forms have been added;

  • Grievance Report Form - multi-page form for initial reporting of a grievance and then sections for documenting the formal review of the submitted report, action taken in response, final outcome and also appendices for documenting statements collected and also any meetings held. This form can be retained as a single form or split into '-A' and '-B' forms if prefer the report and the review to be held separately. The form includes guidance on redaction before sharing (for data protection) and is marked as 'confidential'. This form also includes a new on-form 'Record Retention Summary' which we are about to add to a number of other forms where personal data is collected to assist with ensuring retention is managed effectively by including this summary of retention and protection directly on the form.
  • Reasonable Adjustment Request Form - simple form for employees to formally request an adjustment to the workplace or facilities
  • Staff Timesheet - simple one page staff timesheet (we've supplied plenty of companies with this type of form in the past, not sure why we've never added a timesheet to alphaZ documents!)
  • Staff Exit Interview - simple one page form with a number of fairly open questions to try and elicit useful information and feedback from someone who's heading for the exit door

It's possible we've tried to keep clear of some HR type forms in the past while trying focus on more general forms but we're often asked for forms like these and having reviewed some of low quality documents supplied by HR companies which were often not very well designed, lacked issue control and information-classification (or were always marked 'confidential' even when they weren't) we have decided we are going to be expanding on the range of F-HR documents available.
