Form for completing and documenting monthly Respiratory Protective Equipment (RPE) Inspection and Maintenance Record.

The form is for logging checks of a particular item of RPE on a monthly basis.

The details of the RPE are detailed including who issued to, date issued and description make / model followed by a table layout with editable details of all the checks required to be completed with default data included on the form providing an overview of typical RPE checks / maintenance (which can be edited to suit);

For each of the checks there is then a section of text detailing further notes and guidance on the check to be completed.

• Field to detail maintenance completed (to meet any legal requirement to document and retain maintenance records) and comments

• Details of who completed the monthly check

The form then has a section at the end for notes and and also a retention statement (it may be a legal requirement to retain completed records).
